DAHDI Config Loss

I ran into a very unusual issue the other day. We had a customer that had their FreePBX instance lose its DAHDI configuration for its PRI card (Digium TE133). We would poll the status of the card and it would show under the alarms as "UNCONFIGURED". I thought for sure it was a hardware issue or something similar. We eventually reset the configuration on the card and boom, it starts working...

Has anyone ran into this before? If so, why the heck does it happen (if you know)?
